    To use Internet registration (below) you have to:

• be a registered center by the study coordinators.
• have a valid userid and password.
• have a valid operator code for each individual entering data.

    Please note the following:

• There are three different reporting forms.
• a) Registration of a new patient to the study
• b) Continuing registration of a patient previously entered to the study
• c) Discharge of the patient from the study (normally after ICU discharge, but the form is also used for premature termination of the patient from the study)
   
• All red fields are mandatory.
   
• The data you enter will be added to the database first after you have pressed the SUBMIT-button.
   
• You can switch to the other forms by clicking on NEW, REPORT och DISCH fron inside any of the forms.
   
• A miniversion of study description is available from each form by clicking on the questionmark.
